Fail2ban jail not working - why?

Hello,
I've posted this on serverfault but got no responses so far:

More info can be found there, but the basic gist is that I have a fail2ban jail called apache-noscript (this is a built in jail, not one I've written myself) and there are at least two entries in my logs that I think should have triggered it but haven't. Any ideas why?
